Chronic Care Management companies join forces to drive innovation in care management

The acquisition represents CoachCare’s eighth acquisition since 2023. Founded in 2019, VitalTech’s unique software and AI expertise compliments CoachCare’s suite existing suite of RPM, Chronic Care Management and other services.
